- Then, search Microsoft forms and select the Microsoft Form web part under the Search results.

- Once, the Microsoft Forms web part is added where we select two ways to create a form. Such as:
- New form
- Add existing form

How to Create a SharePoint Microsoft New Form
If you want to create a new form from the SharePoint Microsoft Forms then, follow the below steps.
- Open the SharePoint Microsoft Forms web part, and click on the New form button like button.

- Then, in the Microsoft Forms pane, provide the new form name (Travel Requests) and click on the Create button as shown below.

- Once, click on the create option it will redirect you to the Forms app, and here, click on the + Add new button to add different data types such as Choice, Text, Rating, and Date. Then, create the form by using these data types.



- When you have completed the new form, go back to your SharePoint Microsoft Forms web part. And, make sure to select the Collect responses option then, click on the OK button. Then, the New form will be added like below.

Note:
Whenever we create a new form by using Microsoft Forms Web part, by default, the web address will be populated in Form web address field.

- Now, you can paste the link under the Form web address, select the Collect responses button, and click on the Ok button. Then, the Existing form will be added as shown below.

- Once, click on the Edit current form option it will redirect to the Forms page. Then, change the Share and collect responses as per your needs like below.

- Also, if you want to show your form results then, click on the Show form results button, and select the Ok button. Then, you will get all the responses.
